Update on accessing GP appointments – Total triage from 1st of July 2025

Thank you to everyone who took the time to provide feedback on our proposed changes to how appointments are requested.   We have listened carefully and understand that many of the concerns raised were around access – particularly for those who may find online systems more difficult to use.

We want to reassure all patients that we are not removing the option to contact us via telephone.  If you are able to use the online system, we kindly ask that you request your GP appointment using the link, as this helps us manage demand more efficiently and allows our phone lines to remain open for those who need them most.

We recognise that some of our elderly or more vulnerable patients may not find the online system suitable.  Please rest assured, you can still call us, and we will support you in getting the care you need.  We are not creating barriers  to access – our goal is to improve how we prioritise and manage appointments so we can provide safe and timely care for everyone.

The feedback has helped us ensure that the changes we are making are fair and inclusive.  Having considered this, and after making it clear that telephone access remains available, we will be going ahead with our plan to implement total triage from the 1st of July 2025.

How do i use it?

Step 1

Access the link here: Contact us about your request

Step 2

Choose your request type

  • Admin request [Fit notes, test results and repeat prescriptions]
  • Health problem [Contact us about a new or ongoing health problem]
  • Read advice online [This takes you to NHS links on conditions, symptoms and treatments]
  • I want to self refer [Find local services you can access without a GP referral]

 

If you choose admin request

  1. Select who the request is for [you or someone else]
  2. Confirm that you are not contacting us about an emergency
  3. Select what you wish to contact us about [Doctors letter, fit note, referral follow up, repeat prescription, test results or something else]
  4. Enter the information it requests
  5. Enter your personal details and preferred contact route.
  6. You can choose to verify yourself or not, this does not matter as we will have your details on our clinical system to match up.
  7. Submit request

 

If you choose health problem

  1. Select who the request is for [you or someone else]
  2. Confirm that you are not contacting us about an emergency
  3. Answer the questions on the form
  4. Enter your personal details and preferred contact route.
  5. You can choose to verify yourself or not, this does not matter as we will have your details on our clinical system to match up
  6. Submit request

 

If you choose read advice online

This will take you to the NHS website where you can search for your problem

 

If you chose self refer

  1. Choose the service you wish to self refer to

 

At no time does this service store your personal details, the details are sent directly to our clinical system where they are stored securely.

This service does not charge you for using it.

 

For same day appointment requests if you do not wish to submit a form online, you can still call our reception team on 01253 204672

 

Please note, if you do not have access to the internet or are not confident doing so.  Please contact us as normal via telephone and our reception team will fill the form out for you.
